Lip fillers: swelling, healing & aftercare

A lip filler treatment with hyaluronic acid is done quickly – but the days afterwards co-decide how you feel and how the result turns out. Here you’ll find what swelling is normal, how long healing takes, what to pay attention to, and which (rare) warning signs need medical attention.

How long are the lips swollen?

Swelling is part of it – lips react sensitively. The typical course:

  • Day 0–1: the strongest swelling, sometimes with a feeling of tightness. The lips may look larger and a little uneven.
  • Day 2–3: the swelling subsides clearly; occasionally it lasts a little longer.
  • Day 10–14: the tissue has fully settled – now the final result shows.

Important: don’t judge your lips on the first day. What looks like “too much” at first is often the swelling, not the filler.

Swelling or bruise – what’s the difference?

Swelling affects the whole lip and comes from the tissue reaction. A bruise (haematoma) is a small, localised blue mark at an injection site. Both are harmless and temporary. Bruises can be reduced by avoiding – where medically appropriate – alcohol, blood thinners and anti-inflammatory painkillers for a few days beforehand. After 24 hours they can be covered well with make-up.

Aftercare: what helps in the first days

  • Cooling (with breaks, never directly on the skin) eases swelling and tightness.
  • For the first one to two days, avoid sauna, solarium, intense sun, strenuous sport and alcohol – heat and circulation promote swelling.
  • Salty food can increase swelling; plenty of water helps.
  • Do not massage or knead the lips firmly in the first days.
  • Sleep slightly elevated on the first night.
  • Skip decorative lip make-up on the treatment day to keep the injection sites clean.

Cold sores: why prophylaxis matters

A lip filler treatment can trigger a herpes outbreak in predisposed people. Therefore:

  • With an active cold sore we do not treat – only after it has fully healed.
  • Anyone who gets cold sores frequently is usually given preventive antiviral therapy (e.g. aciclovir) around the appointment, after medical assessment.

So be sure to mention a tendency to cold sores before treatment – it is part of the medical counselling and is reflected in the contraindications.

When does the result settle – and when may it be corrected?

Small bumps, palpable lumps or slight asymmetry in the first days are normal and usually even out on their own as the swelling subsides. The tissue settles over about ten to fourteen days. Only after that can it be judged whether a fine-tuning makes sense – not before. One advantage of hyaluronic acid: it can be dissolved on purpose with the enzyme hyaluronidase if needed.

Warning signs: when to contact us immediately

A vascular occlusion after a properly performed lip filler treatment is very rare – but you should know the signs, because acting quickly matters. Contact us immediately for:

  • unusually severe, increasing pain that doesn’t fit the normal swelling,
  • a whitish blanching or bluish, blotchy discolouration of the lip or surroundings,
  • increasing discolouration that spreads.

In such a case the filler can be dissolved quickly with hyaluronidase – another reason the treatment belongs in medical hands that keep the emergency antidote on hand. When in doubt: better to ask once too often.

Conclusion

Swelling, a feeling of tightness and small bruises are part of a lip filler treatment and settle within a few days; the final result shows after one to two weeks. With the right aftercare – cooling, taking it easy, herpes prophylaxis – healing is usually uncomplicated. Learn more about the procedure and risks on the hyaluronic-acid fillers page.

Frequently asked questions

How long are lips swollen after fillers?

The strongest swelling is in the first 24 hours and clearly subsides afterwards. Occasionally it lasts two to three days. The final result can only be judged after about ten to fourteen days, once the tissue has fully settled.

What should you avoid after a lip filler treatment?

For the first one to two days, avoid sauna, solarium, intense sun, strenuous sport and alcohol, and do not massage the lips firmly. Cooling eases the swelling; salty food can increase it. Sleeping slightly elevated on the first night also helps.

Can you get lip fillers despite herpes?

With an active cold sore we do not treat – only after it has fully healed. Because the injection can trigger an outbreak, patients who get cold sores frequently are usually given preventive antiviral therapy (e.g. aciclovir) around the appointment, after medical assessment.

How long does it take for lip filler to settle?

Small bumps, a feeling of tightness or slight asymmetry in the first days are normal and usually even out on their own. The tissue settles over about ten to fourteen days. Only after that – not on the first day – should the result be judged or any fine-tuning discussed.

When should I see a doctor after a lip filler treatment?

Contact us immediately for unusually severe or increasing pain, or a whitish or bluish, blotchy discolouration of the lip or surrounding area. These can – very rarely – be signs of a vascular occlusion, which can be treated quickly with the enzyme hyaluronidase. When in doubt, always have it checked.

How do I reduce bruising after the treatment?

Small bruises are possible and harmless. They can be reduced by avoiding – where medically appropriate – blood thinners such as alcohol and anti-inflammatory painkillers for a few days beforehand. Cooling right after the treatment and covering with make-up after 24 hours also help.
