How does a mouth-corner lift with filler work?
Hyaluronic acid is placed precisely beneath the downturned mouth corner and the adjoining area. It supports the tissue from below, replaces lost volume and softens the downward pull. This makes the corner look lifted and the expression less “sad”. The result is visible immediately and dissolvable if needed.
Downturned mouth corners arise from several factors together: volume loss in the lower face, declining skin tone and the pull of the muscle that draws the corners down (depressor anguli oris). With hyaluronic acid we mainly address the volume component and support the corner from below.
We often treat the corner not in isolation but include the adjoining marionette region and – where useful – the chin. Only this interplay creates a stable, harmonious effect rather than just filling a single point.
If the downward muscle pull is very pronounced, complementary relaxation of that muscle with botulinum toxin can make sense. Whether hyaluronic acid alone is enough or a combination gives the more natural result, we discuss honestly in the consultation.
We dose deliberately conservatively: the goal is a subtly friendlier expression, not a changed face. Around the mouth in particular, too much volume can quickly look unnatural.
How the treatment works

The effect can then be refreshed.

- Common and temporary: swelling, redness, tenderness or small bruises around the mouth corner.
- Occasionally: palpable unevenness or slight asymmetry, usually easy to adjust.
- Reactivation of lip herpes is possible in the mouth region; with a known tendency, preventive treatment may help – please mention it.
- Very rare but serious: vascular occlusion. For this emergency we keep the antidote hyaluronidase and an emergency protocol ready and work, where possible, with blunt micro-cannulas; hyaluronic acid is also dissolvable.
- Contact us immediately in case of severe or increasing pain, pale/blotchy skin discolouration or changes in vision.
When should treatment be avoided?
- Pregnancy and breastfeeding
- Active infection, inflammation or cold sores in the mouth area
- Known allergy to hyaluronic acid or components of the gel
- Tendency to keloids or an active autoimmune condition in an inflammatory phase – assessed individually
- A very strong downward-pulling muscle (depressor anguli oris) or pronounced skin laxity rather than volume loss – here hyaluronic acid alone is often not enough; a combination with botulinum toxin or a different approach is more suitable
Aftercare
- Do not massage the treated area firmly for 24–48 hours (unless instructed)
- No intense sport, sauna or tanning for the first 24 hours
- Cooling eases swelling; alcohol and blood thinners promote bruising
- Avoid vigorous mouth movements and make-up on the area on the first day where possible

Can downturned mouth corners be a sign of illness?
Usually they are a harmless result of volume loss and declining skin tone. A sudden, one-sided droop – often with other symptoms – should instead be assessed medically (e.g. a nerve palsy). Such cases are not for aesthetic treatment; we refer you appropriately.
What should I keep in mind after a mouth-corner lift?
