With age the mouth corners often drop, making the face look sad or grumpy at rest even when you feel fine. By precisely supporting the corner and the adjoining marionette region, the resting expression can look friendlier and more relaxed again.
How does a mouth-corner lift with filler work?
Hyaluronic acid is placed precisely beneath the downturned mouth corner and the adjoining area. It supports the tissue from below, replaces lost volume and softens the downward pull. This makes the corner look lifted and the expression less “sad”. The result is visible immediately and dissolvable if needed.
Downturned mouth corners arise from several factors together: volume loss in the lower face, declining skin tone and the pull of the muscle that draws the corners down (depressor anguli oris). With hyaluronic acid we mainly address the volume component and support the corner from below.
We often treat the corner not in isolation but include the adjoining marionette region and – where useful – the chin. Only this interplay creates a stable, harmonious effect rather than just filling a single point.
If the downward muscle pull is very pronounced, complementary relaxation of that muscle with botulinum toxin can make sense. Whether hyaluronic acid alone is enough or a combination gives the more natural result, we discuss honestly in the consultation.
We dose deliberately conservatively: the goal is a subtly friendlier expression, not a changed face. Around the mouth in particular, too much volume can quickly look unnatural.
How the treatment works

Results & longevity
The lifted corner is visible immediately; once mild swelling subsides (usually a few days) the final result shows. In this highly mobile mouth region, hyaluronic acid lasts about 8 to 12 months depending on metabolism – tending to be a little shorter than in calmer areas such as the chin or jawline. The effect can then be refreshed.
Results are individual and may vary. Before/after images show real treatments and are published only with the patient’s explicit consent.
Risks & side effects
Risks & side effects
- Common and temporary: swelling, redness, tenderness or small bruises around the mouth corner.
- Occasionally: palpable unevenness or slight asymmetry, usually easy to adjust.
- Reactivation of lip herpes is possible in the mouth region; with a known tendency, preventive treatment may help – please mention it.
- Very rare but serious: vascular occlusion. For this emergency we keep the antidote hyaluronidase and an emergency protocol ready and work, where possible, with blunt micro-cannulas; hyaluronic acid is also dissolvable.
- Contact us immediately in case of severe or increasing pain, pale/blotchy skin discolouration or changes in vision.
Aftercare
- Do not massage the treated area firmly for 24–48 hours (unless instructed)
- No intense sport, sauna or tanning for the first 24 hours
- Cooling eases swelling; alcohol and blood thinners promote bruising
- Avoid vigorous mouth movements and make-up on the area on the first day where possible

Filler or Botox for downturned corners?
Hyaluronic acid supports the corner with volume; botulinum toxin relaxes the muscle pulling it down. A combination is often the most natural – we assess this individually.
