Tear trough & under-eye filler with hyaluronic acid in Wolfsburg
The tear trough under the eye can be treated with hyaluronic acid at our Wolfsburg practice to fill sunken shadows and tired-looking under-eye circles. This area is especially delicate: the skin here is very thin and the anatomy demanding. How does tear-trough treatment work?
A small amount of a soft hyaluronic-acid gel is placed deep in the tear trough, usually with a blunt cannula. It fills the sunken groove, softens the dark shadow and smooths the transition to the cheek. Deliberately dosed sparingly, because the thin skin here reacts very sensitively.
Under-eye circles have various causes – not every one can be fixed with filler. If the cause is a sunken tear trough (a true volume deficit), hyaluronic acid can noticeably soften the shadow. For dark circles caused by pigmentation or vessels showing through, filler helps little. We distinguish this carefully in the consultation.
The tear trough is one of the most demanding and highest-risk filler areas. The skin here is extremely thin, and the proximity to the eye and to vessels demands particular caution. We place the gel deep and sparingly, preferably with a blunt micro-cannula, and use a soft, low-water-binding product to keep swelling minimal.
An honest suitability check is essential: not everyone is a candidate. With pronounced eye-bags, herniated lower-lid fat (fat herniation), marked skin laxity or a tendency to swelling (e.g. lid oedema, malar bags/cheek puffiness), filler can worsen the result. In such cases other approaches – for example surgical lower-lid correction – are often the better choice, and we say so openly.
If the gel is placed too superficially or in too large an amount, it can show through as a bluish tint (Tyndall effect) or cause persistent swelling. This is exactly why we dose particularly conservatively here; if a result is unwanted, the hyaluronic acid can be specifically dissolved again with hyaluronidase.

Results & longevity
An initial lightening of the shadow is often visible immediately. Because the tear trough can swell slightly at first, the final result only appears after about one to two weeks. As the area is very calm, hyaluronic acid usually lasts a long time here – about 9 to 18 months. We build the volume up slowly on purpose to avoid overfilling.

- Common and temporary: swelling and small bruises – in the thin-skinned lower-lid area often more noticeable and longer-lasting than elsewhere.
- Typical for this area: persistent swelling or under-eye “puffiness”, or cheek swelling (malar oedema), especially with too much or too superficially placed gel, or with a corresponding predisposition.
- Bluish tint (Tyndall effect) when placed too superficially; it usually does not resolve on its own but can be dissolved with hyaluronidase.
- Occasionally: palpable nodules or irregularities in the thin skin.
- Very rare but serious: vascular occlusion. Near the eye this is particularly critical, as through vascular connections, in very rare cases, visual disturbance up to vision loss is possible. We therefore treat preferably with a blunt cannula, very slowly and in small amounts, and keep the antidote hyaluronidase plus an emergency protocol ready. Contact us immediately in case of sudden pain, changes in vision or pale/blotchy skin.
Aftercare
- Do not rub or massage the lower-lid area for 48 hours (unless instructed)
- No intense sport, sauna or tanning for the first 24 hours; keep your head slightly elevated at night
- Careful cooling can reduce swelling and bruising; do not press firmly
- Eating low-salt and drinking enough can reduce swelling around the eyes

Suitable for a true, sunken tear trough with good skin quality and no tendency to swelling. Not everyone is a candidate: with pronounced eye-bags, herniated lower-lid fat or marked skin laxity, other approaches are often more appropriate – we assess this honestly before treating.
